4/17/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Jahsim. Upon arrival to the home, he verbalized to QMHP-C that he was not in a good mood. QMHP-C probed Jahsim to determine what was going on with his day. He stated that his siblings were getting on his nerves, his grandmother was getting on his nerves and his mother. QMHP-C engaged in a discussion with Jahsim on how he needs to show leadership and become more responsible as it relates to his siblings and others in the home that he needs to respect. Ms. Shabazz began to explain to QMHP-C on how Jahsim got upset yesterday and began to curse at her, throw things in the house and state what he was and what he was not going to do. Ms. Shabazz verbalized to QMHP-C that she doesnât know what to do with him anymore and she is glad that he got a counselor to try and help him. 2. Behavior: Jahsim participated in the session and provided feedback as it related to what was being discussed throughout the session; however, he did become frustrated with counselor due to some of the behaviors counselor was discussing with him (per his mother). 1.Intervention: QMHP-C began the session by assessing Jahsimâs emotional well-being. QMHP-C reviewed the recent incident that occurred in the home. QMHP-C allowed Jahsim the opportunity to implement self-calming strategies prior to the sessionâs beginning. QMHP-C immediately confronted Jahsimâs aggressive behaviors that he has been presenting in the home. QMHP-C processed with Jahsim on how he needs to be a role model for his those in the home and show some respect towards those who are in authority. QMHP-C encouraged grandmother to confront his behaviors in a stern manner. QMHP-C modeled the appropriate way for Ms. Shabazz to confront behavior. 2.Intervention: QMHP-C engaged Ms. Shabazz and Jahsim in a discussion to allow them both to identify triggers that resulted in the situation. QMHP-C assisted Jahsim and Ms. Shabazz with identifying areas that could have been avoided to prevent his behavioral situations. QMHP-C provided them with alternative ways to manage conflicts. QMHP-C shared with Jahsim possible consequences for his behavior exhibited towards his family members as they have been to court several times within the last month for his aggressive behaviors. QMHP-C asked Jahsim to demonstrate the appropriate ways to respond to certain situations within the home that he doesnât like. 1.Response: Jahsim was receptive to suggestions that were provided to him. Jahsim was visibly upset when QMHP-C arrived to the home. He was yelling and screaming at his mother and mother. Ms. Shabazz was also visibly upset and frustrated with Jahsimâs behavior. Jahsim had limited communication with his mother. He communicated with QMHP-C but showed little remorse for his actions and aggressive behaviors towards his mother. Response: During the session, Jahsim blamed shift his reactions and minimize his behaviors. Ms. Shabazz struggles to confront Jahsim appropriately. Ms. Shabazz becomes provoking as she confronts Jahsimâs behaviors due to the frustration that she has regarding Jahsimâs constant negative/aggressive behaviors. Jahsim displayed a lack of interest as QMHP-C was discussing with him positive ways to address situations that occur within the home.
